After completing the registration information by setting up your unique username and password, we will activate your account.

For your convenience, all members of a family will be added to a single portal account. Your request to activate a portal account requires your informed consent to open a family account and all current members of your family will be added to the account during the activation process.
If you prefer to keep your information separate, please do NOT set up an online family portal account.
The Family Portal can be used to:
Update forms. Forms will be available in the portal, pre-populated with your prior information 2 weeks before an appointment (similar to filling out forms on an iPad in our reception area).
View appointment dates and times.
View balances and make online payments.
Please note that we are a fee for service practice and insurance information (estimates etc.) are not accurate on the portal. Although we are happy to submit insurance forms for you at the time of your visit and assist you in receiving maximum benefits, patients should contact their individual insurance companies for information about their unique dental health benefits.
