Express Check-In

Prior to Your Appointment:

New patients should complete the New Patient Online Questionnaire as soon as possible prior to their appointment, a dental insurance form if applicable, and a COVID-19 Health Screening Form.
Existing patients will complete medical history update forms in person during their scheduled appointment.

Upon Arrival for Your Appointment:

Please park in our attached parking lot (entrance on Valley Road to the left of the office) and call the office to let us know you have arrived (973-744-3181).
